| Objective: | Provides counseling, education and case management services to chemically dependant patients. Quality of work, performance expectations and responsibilities will be conducted in a manner that; is both consistent with current standards of practice and updated to incorporate current research trends and, is consistent with the organization's mission statement, goals and objectives. |
| Qualifications: | Degree in behavioral science, Master's preferred; experience working with chemically addicted patients, must demonstrate a positive non-judgmental and accepting attitude toward the chemically dependent patient. |
| Responsibilities: | Conducts individual initial assessment and evaluation interviews for service eligibility determination, including urine specimen collection. Facilitates, establishes and reviews with each patient the initial treatment plan w/in 30 days. Completes Bio-psychosocial assessment w/in 30 days. Conduct a monthly treatment plan review for patients accepted into the medically supervised withdrawal program. Conduct a quarterly treatment plan review for patients accepted into the maintenance program. Provide individual and group counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment. Involve families and significant others in patient's recovery process whenever possible. Develops and participates in individual or group patient teaching projects. Respond, as needed, to patient grievances and complaints. Develop aftercare and discharge plans. Completes all discharge paperwork and closes out active chart w/in established timeframe. Other related duties as determined by supervisor. |
| Administrative: | Participates in department meetings, committees and quality improvement initiatives. Participates in weekly case review with team members. Assist in quality assurance chart reviews, assessing clinical and medical documentation to ensure compliance with regulation and program standards. Meets all billing and paperwork requirements. Maintains prompt and courteous relationships with Managed Care Providers insuring continued coverage, completes all required paperwork w/in stated deadlines. Participates in teaching of other nursing and ancillary personnel and students. Establish discharge linkages. Cooperate with all program staff and other community agencies. Maintains current knowledge of developments in the substance abuse treatment field and is responsible for self-development through continuing education programs. |
